First Visit

 You can come to your first appointment without completing any of the following beforehand.  However, you can review the following and complete the tasks prior to your first appointment if you’d prefer.

Medical Records

Please request a copy of your child’s medical records.

 

• You can request a digital copy of the file on a CD or DVD.
• You can have them mailed to our office at 1010 Lawrence Street, Brandenburg, KY 40108.
• You can pick them up from your current physician in person and bring them to our office.
